FEF Grant Forms
Click on the button below to download the budget form you will submit with your Project Grant application. This form is not for mini-grants, which do not have a separate form.
Remember you must rename the form with the title of your project then upload the renamed form to your application, where indicated.
Those requesting reimbursements for expenses related to any grant or payment for a presenter (visiting artist, speaker, author etc.) will need to complete the payment request form.
Please scan or take a photo of your receipt. We would appreciate you submitting receipts and invoices within 6 weeks.
Tax paid by grantees cannot be reimbursed.
If invoices have a due date please submit as soon as possible.

All grant recipients need to complete an assessment form or report, which should be submitted by June 1.
